Information collected

Tandem Physio may collect referrer details, client contact and location details, funding pathway, broad clinical reason for referral and relevant consent information. Detailed clinical records should only be shared where necessary and with appropriate consent.

How information is used

Information submitted through the referral page is used to respond to the enquiry, determine service suitability, arrange appointments where appropriate and communicate with authorised people involved in the client’s care.

Referrer responsibility

Referrers should confirm that the client or authorised representative has consented to the referral and to relevant information being shared with Tandem Physio.

Minimise sensitive information initially

Please avoid sending unnecessary sensitive information at the first enquiry stage. Further reports, plans or clinical documents can be requested once consent, funding and service arrangements are confirmed.
